Not sure what Mom or Dad needs?

Most families start with one conversation. Tell us what a typical day looks like now — and what it looked like a year ago — and we will tell you honestly what level of care fits.

What Families Call Us About

The moments that make staying home feel uncertain

You usually do not go looking for senior care. Something happens — a fall, a discharge, a phone call from a neighbor — and suddenly the house is not as safe as it was.

After a fall

Balance, transfers, bathroom trips and stairs — the highest-risk moments of an older adult's day. We staff for them and keep the path through the house clear.

Home from the hospital

The first weeks after discharge can be some of the most challenging. Medications, wound care, follow-up appointments and someone watching for what changes.

Memory changes

Support for older adults living with dementia or Alzheimer's — familiar routines, calm redirection, and a caregiver who stays consistent week to week.

Medications piling up

Reminders from a caregiver, or administration and management by a licensed nurse when the regimen is beyond a pill organizer.

Too much alone time

Isolation takes a real toll on older adults. Sometimes what a parent needs most is company, a meal eaten with someone, and a reason to get dressed.

A family caregiver running out

If you have become the caregiver, respite is not a luxury. Hand over the shift — for an afternoon or a fortnight — and go be the son or daughter again.

Our Foundation

12WELVE STONES.
One foundation.

Our name remembers the twelve stones set down as a memorial — a marker meant to be remembered, and a reminder that something lasting is built one stone at a time.

That is how we think about caring for an older adult. No single visit changes a life. But a caregiver who arrives on time, a nurse who catches a change early, a hand under an elbow on the way to the bathroom — laid one on top of another, those become a foundation a family can stand on. That is what Compassionate Care. Stronger Together. means to us.

We serve seniors across Snellville, Gwinnett County and Metro Atlanta, from a few hours of company a week to around-the-clock support after a hospital stay.

More about us
How It Works

Getting started is simple

No obligation, no pressure, and no long intake maze. Four steps from first call to first shift.

01

Free consultation

A short conversation to understand your parent's needs, routine, preferences and what worries you most.

02

In-home assessment

We meet you and your loved one at home to review health needs, daily routines, and the places the house has become risky.

03

A care plan built for one person

We put the plan in writing, match a caregiver to the person — not just the shift — and introduce them properly.

04

Care begins — and keeps adjusting

We supervise, stay in touch, and revise the plan as needs change. You are never guessing how it is going.

Common Questions

Answers before you call

If your question is not here, ask it — we would rather have the conversation than have you guess.

Ask a question
My mother does not want a stranger in her house.
That is the most common thing we hear, and it is a fair objection. We start small — a few hours, one task she has actually asked for help with — and we keep the same caregiver so it stops being a stranger. Most families find the resistance is about losing independence, not about the person.
How quickly can care start?
In many cases within a few days of the assessment, and sooner for urgent hospital discharges. Call us and we will tell you honestly what we can staff and when.
What is the minimum number of hours?
We build schedules around what a family actually needs, from a few hours a week to around-the-clock care. Minimums vary by shift type — we will go over them on your consultation call.
Do you provide care on weekends and overnight?
Yes. Our team staffs weekdays, weeknights and weekends, including overnight shifts and live-in arrangements — the hours that are hardest for working families to cover.
Can you care for someone with dementia?
We support older adults living with dementia and Alzheimer's, with an emphasis on familiar routines, calm redirection and caregiver consistency. Call us to talk through the stage and behaviours involved so we can tell you honestly whether home care is the right setting.
I live out of state. How will I know how Mom is doing?
You get regular updates and a direct number to call. Distance should not mean finding out a month late that something changed.
How do you handle payment?
We work with private pay and long-term care insurance. We will walk you through rates and what your policy covers before you commit to anything.
