12WELVE STONES HOME HEALTHCARE, LLC is an in-home senior care agency serving families across Snellville, Gwinnett County and Metro Atlanta — pairing vetted caregivers, CNAs and nurses with the older adults who need them.

Our Name

Why 12WELVE STONES?

Our name remembers the twelve stones once set down as a memorial — a marker placed so that when children asked what it meant, there would be a story to tell. Twelve separate stones, standing together as one.

We took that name because it says what we believe about caring for an older adult. No single visit changes a life. But a caregiver who shows up on time, a nurse who catches something early, a note passed to the family at the end of a shift — laid one on top of another, those become a foundation a family can stand on.

That is what our promise means: Compassionate Care. Stronger Together.

Quality

The older adult in front of us comes first on every visit and every shift. We measure ourselves by what actually happens in the home, not by what is written in a plan.

Respect

We uphold the dignity of every person in our care. An 80-year-old is still the adult who raised a family and ran a household. Independence is not something to be managed away — it is something to be protected for as long as possible.

Handing over the care of a parent is an act of trust, and trust should be checkable. Here is how we operate.

  • Built around Georgia DCH standardsOur policies, caregiver screening and documentation are built around Georgia Department of Community Health standards for private home care providers.
  • Credential verificationCNA registry status, nursing licenses, required credentials and references are verified before placement and monitored as applicable.
  • Background screeningCriminal background checks are completed on every care professional we place in an older adult’s home.
  • Supervision and oversightCases are supervised, documented and reviewed — not handed off and forgotten.
  • ConfidentialityClient health information is handled confidentially and shared only as permitted by law and as needed to deliver care.
